Material details
1. Description of material: (eg. photograph, film-footage, badge/crest/logo, text etc, along with the relevant MOD / archive / library reference. The reference is required to enable a check on the copyright status).
2. Source of material. Please also provide the date of the material if it is not from the Defence Image Database.
Multiple use licence (photos)
Please advise the length of the period that you require a multiple use photo licence for (ie: 1 or 3 years), if appropriate. You need not complete this form any further.
Proposed use: (eg. personal, non-commercial, research, educational, reproduction in a publication, book, advertisement, marketing material (please specify), first day covers, cards, posters, film/documentary, video, DVD, print media, website).
Miscellaneous
1. If the material is to be used on a front cover of a publication please advise.
2. Please advise the name of the publication.
3. Please advise the number of copies to be produced where applicable.
4. If the material is to be reproduced on a client's behalf please advise their name.
5. If the material is to be reproduced in an advertisement please advise which publications it is to appear in.
6. If the material is to be used on a website please advise over what period of time.
