JSATFA Forms

These forms are to be used by RN and RAF personell wishing to submit an application to undertake Adventurous Training under the Joint Services scheme, wher the expedition is sponsored by their respective Service.

Note: The forms on thi page may only be used for expeditions which are not sponsored by the Army. For Army sponsored expeditions, the expedition leader must submith an application via ATSYS. If you have no access to a networked MOD computer, you may submit an Offline JSATFA (refer to ATSYS JSATFA Forms under related links on the right).
