Joint Service Adventurous Training Sub-Aqua Diving Application Form

Applicants who are offered a place on the British Sub Aqua Club (BSAC) Ocena Diver (ODC) course will be sent a 'Fit to Dive' certificate that must be signed by your Medical Officer, and returned to Joint Service Sub Aqua Diving Centre (JSSADC) prior to the course. All previously qualified divers attending upgrading courses, (eg BSAC Sports Diver (SPC) and above), will be required to produce thier qualification log books and medical certificates.
