These documents form Parts 2 and 3 of a request for the disclosure of service details of a deceased former member of the Army. Part 2 is for general enquirers and Part 3 is for next of kin enquirers. Part 1 must also be completed in all cases and can be found under "Related Pages", as either a General Enquirers' form or Next of Kin form.

This form (Part 1) is for completion by general enquirers requesting information relating to deceased service personnel. After completing this form, enquirers should use a link under "Related Pages" to go to the relevant service page and complete Part 2.

If you are the next of kin then please use the form provided for requests from next of kin.
